Information for New Patients

For Patients

To initiate treatment at The Psychological Center, prospective patients must complete an online information packet. The online information packet is to be completed by a legal guardian for all prospective patients 17 years and younger (minors).

Once the packet is complete, our Intake Coordinator will call you to schedule an initial Screen Appointment. Screen Appointments are conducted in-person only. Individuals and/or legal guardians (parents/caregivers) of minors interested in receiving psychological or neuropsychological testing must attend their screen appointment in-person.

The Psychological Center provides in-person services only. All prospective patients must present an ID to CCNY Security at the building entrance when they come to our Clinic.

For Providers

Providers should instruct potential patients to call for an appointment. Providers cannot schedule this appointment for the potential patient. Please call 212.650.6602 if you have any questions.
